Job Description

The individual in this position is responsible for the psychosocial assessment of patients, for post-hospital discharge needs including home care, nursing home placement, durable medical equipment, financial assistance, counseling, and other community resources. Ensures appropriate decision makers are informed of and involved in treatment planning. Responsible for facilitating the Medicare Notice of Discharge to identified patients and perform multiple activities that support a seamless transition of patients from the inpatient hospital setting, using current knowledge of hospital processes and community services.

POSITION QUALIFICATIONS: Bachelor’s Degree in Social Work, Psychology, or related field from an accredited school. One (1) year of hospital Social Work experience or two (2) years of nursing home experience. Agency Affiliated Counselor certification through the state of Washington. Basic Life Support Heartsaver (HS) level to be completed within three (3) months of hire
